Providence E-Card 1.4

Thank you for downloading the Providence Health Plan E-Card app. This application provides a convenient way to access your ID card and manage related information. With the app, users can:

• View their own ID card
• Access the ID cards for their spouse and/or dependents covered under the policy
• Email or fax ID cards to medical providers such as doctors' offices, hospitals, or pharmacies

To gain access to these features, follow these steps:

1. Download the application
2. Enter your member ID number
3. Input your 4-digit PIN; you can find this number on the back of your ID card (lower left-hand corner). If you do not have your PIN, kindly request it from Providence Health Plan Customer Service at 503-574-7500 or 800-878-4445.
